We recently learned that records associated with the practice had been improperly discarded during a records clean-out. After members of our team became aware of the situation, the materials were promptly retrieved and secured.

We immediately initiated a comprehensive review to determine the scope of the incident, understand exactly what information was involved, and begin the appropriate reporting process with regulatory authorities, consistent with applicable requirements.

We are continuing to evaluate the circumstances surrounding this incident, including the types of information involved and whether any additional notifications are required. We are also reviewing our records management and document disposal procedures and implementing additional safeguards to help prevent a similar incident from occurring in the future.

We understand that situations involving personal information can be unsettling. We sincerely apologize for the concern this incident may cause. Protecting the privacy of our patients and employees is a responsibility we take very seriously, and we are committed to handling this matter with transparency, accountability, and care.
